North Bay Parry Sound District Health Unit

The North Bay Parry Sound District Health Unit is a Canadian provider of health services under the auspices of the Government of Ontario.

During the COVID-19 vaccine rollout, the health unit distributed a “COVID-19 Vaccine Screening and Consent Form” provided by the Ontario Ministry of Health, as well as an accompanying “COVID-19 Vaccine Information Sheet”. Both documents underwent numerous revisions between December 2020 and October 2022.
